Services for Long Term Medical Condtion Management


Over the last few months St Helens Commissioners and the Long Term Conditions Expert Patient Group have been working hard to develop ideas that will support people better in the community to self-manage their Long Term Condition.

Now, they are gathering some views and information on what kind of service a person with a Long Term Medical Condition would attend to help them self- manage their condition.   This survey is just mapping the general views of patients and the public and will be used to further inform commissioners and the Expert Patient Panel with a view to supporting St Helens Clinical Commissioning Groups priorities around: -

1.      Improved early detection and management of long term conditions
2.      Increased access to education for patient, public and professionals
3.      A holistic approach to care closer to home
